    Modern Narcissus: the lingering reflections of myth in modern art

    Why has myth continued to fascinate modern artists, and why the myth of Narcissus, with its modern association with narcissism? This article considers the relationship between the Narcissus myth and the lineage of modern art that runs from Symbolism to surrealism through the polymorphous prism of the Greco-Roman Pantheon to which Narcissus belongs. The article offers an interpretation of the role of mythology in modern art that moves beyond psychoanalysis to incorporate the longer span of the art-historical tradition. Addressing issues of aesthetics, gender and sexuality, the following account highlights Narcissus‟s double nature as an erotic myth that comprises both identity formation and intersubjectivity, as enacted in the field of representation. The myths associated with Narcissus in the history of Western art will help us reconsider his role as a powerful figure capable to activate that slippage between word and image, identity and sociability, representation and reality which was celebrated by the Symbolists and formed the centre of the surrealists‟ social-aesthetic project

    Detection of Pseudomonas syringae pv. actinidiae, causal agent of bacterial canker of kiwifruit, from symptomless fruits and twigs, and from pollen

    Pseudomonas syringae pv. actinidiae (Psa), the causal agent of bacterial canker of kiwifruit, was monitored in symptomless fruits, twigs and pollen of the host using bacterial isolation and DNA-extraction followed by two PCR-assays (direct-PCRs). A procedure for Psa detection from symptomless twigs was established. Out of 16 symptomless twigs samples, Psa was detected in 12 samples by isolation and 13 samples by direct-PCR. Thirteen pollen samples were treated using two different procedures; Psa was detected in eight samples by isolation and ten samples by direct-PCR. By washing 108 samples of fruits, Psa was detected by isolation in only two samples, collected from severely affected orchards. However, one of these samples contained wilted fruits, whereas for the other, only one colony was isolate. From 60 bulk-samples of fruits, endophytic Psa was detected in six samples by isolation and ten samples by direct-PCRs. A Psa-positive bulk-sample of fruits was analyzed separately as individual fruits: there was a faint signal in five or seven fruits out of 50 depending on the PCR assay used. Isolation was negative for these samples. Presence of the pathogen on bulk-fruit samples could be due to low amounts of inoculum distributed over many fruits: as a consequence, there is a negligible risk of introducing the pathogen into countries free of bacterial canker. This integrated approach (isolation plus PCR) is proposed as a tool for the analysis of symptomless kiwifruit material for the presence of Psa.

    Utilidad de la hormona anti-mülleriana (AMH) y la inhibina B en el diagnóstico del hipogonadismo en el niño

    Durante la infancia, el eje hipotálamo-hipófiso-testicular se encuentra parcialmente quiescente: bajan los niveles de gonadotrofinas y la secreción de testosterona disminuye siguiendo a la caída de la LH. Por el contrario, las células de Sertoli están activas, como lo demuestran los niveles séricos de hormona anti-mülleriana (AMH) e inhibina B. Por lo tanto, el hipogonadismo en la infancia puede ser puesto en evidencia, sin necesidad de pruebas de estímulo, si se evalúa la función de las células de Sertoli. La AMH sérica es alta desde la vida fetal hasta el inicio de la pubertad. La producción testicular de AMH aumenta en respuesta a la FSH pero es potentemente inhibida por los andrógenos. La inhibina B es alta en los primeros años de la vida, luego disminuye parcialmente aunque permanece claramente más alta que en las mujeres, y aumenta nuevamente en la pubertad. Las concentraciones séricas de AMH e inhibina B son indetectables en pacientes anórquidos. En el hipogonadismo primario que afecta a todo el testículo, establecido durante la vida fetal o la infancia, todos los marcadores testiculares están bajos. Cuando en el hipogonadismo están afectadas sólo las células de Leydig, la AMH y la inhibina B sérica son normales y/o altas, mientras que están bajas cuando se ven afectadas las células de Sertoli. La AMH y la inhibina B están bajas en varones con hipogonadismo central en edad prepuberal y continúan bajas en edad puberal. El tratamiento con FSH induce un aumento en los niveles séricos de los marcadores de la célula de Sertoli. En conclusión, la determinación de los niveles séricos de AMH e inhibina B es útil para evaluar la función testicular, sin necesidad de pruebas de estímulo, y orientar el diagnóstico etiológico en el hipogonadismo masculino en pediatría.During childhood, the hypothalamic-pituitary-gonadal axis is partially quiescent: gonadotropin and testosterone levels decrease, but Sertoli cells remain active, as shown by serum anti-Müllerian hormone (AMH) and inhibin B levels. Therefore, hypogonadism may be diagnosed during childhood, without the need for stimulation tests, provided Sertoli cell function is assessed. Serum AMH levels are high from fetal life until the onset of puberty. Testicular AMH production increases in response to FSH but is potently inhibited by androgens. Serum inhibin B levels are high until the age of 3-4 years in boys; although they decrease thereafter, they remain clearly higher than in girls of the same age. During the early stage of puberty, serum inhibin B increases again to reach adult values. AMH and inhibin B are undetectable in the serum of anorchid patients. In boys with fetalonset primary hypogonadism affecting the whole testicular parenchyma, AMH and inhibin B are low in serum. Conversely, they are normal or high when only the interstitial tissue of the gonads is impaired. AMH and inhibin B are low in children with central hypogonadism and persist low during pubertal age. FSH treatment induces an increase in both Sertoli cell markers. In conclusion, the determination of serum AMH and inhibin B levels is useful for the assessment of testicular function, without the need for stimulation tests, in pediatric patients.Fil: Grinspon, Romina.     Challenges of Prevention for a Sustainable Personalized Medicine

    The development and implementation of the approaches of personalized medicine for disease prevention are still at infancy, although preventive activities in healthcare represent a key pillar to guarantee health system sustainability. There is an increasing interest in finding informative markers that indicate the disease risk before the manifestation of the disease (primary prevention) or for early disease detection (secondary prevention). Recently, the systematic collection and study of clinical phenotypes and biomarkers consented to the advance of Rehabilomics in tertiary prevention. It consents to identify relevant molecular and physiological factors that can be linked to plasticity, treatment response, and natural recovery. Implementation of these approaches would open avenues to identify people at high risk and enable new preventive lifestyle interventions or early treatments targeted to their individual genomic profile, personalizing prevention and rehabilitation. The integration of personalized medicine into prevention may benefit citizens, patients, healthcare professionals, healthcare authorities, and industry, and ultimately will seek to contribute to better health and quality of life for Europe\u2019s citizens

    Spreading the clinical window for diagnosing fetal-onset hypogonadism in boys

    In early fetal development, the testis secretes – independent of pituitary gonadotropins – androgens and anti-Müllerian hormone (AMH) that are essential for male sex differentiation. In the second half of fetal life, the hypothalamic–pituitary axis gains control of testicular hormone secretion. Follicle-stimulating hormone (FSH) controls Sertoli cell proliferation, responsible for testis volume increase and AMH and inhibin B secretion, whereas luteinizing hormone (LH) regulates Leydig cell androgen and INSL3 secretion, involved in the growth and trophism of male external genitalia and in testis descent.This differential regulation of testicular function between early and late fetal periods underlies the distinct clinical presentations of fetal-onset hypogonadism in the newborn male: primary hypogonadism results in ambiguous or female genitalia when early fetal-onset, whereas it becomes clinically undistinguishable from central hypogonadism when established later in fetal life. The assessment of the hypothalamic–pituitary–gonadal axis in male has classically relied on the measurement of gonadotropin and testosterone levels in serum. These hormone levels normally decline 3–6 months after birth, thus constraining the clinical evaluation window for diagnosing male hypogonadism. The advent of new markers of gonadal function has spread this clinical window beyond the first 6 months of life. In this review, we discuss the advantages and limitations of old and new markers used for the functional assessment of the hypothalamic–pituitary–testicular axis in boys suspected of fetal-onset hypogonadism.Fil: Grinspon, Romina.     Frailty Network in an Acute Care Setting: The New Perspective for Frail Older People

    The incidence of elderly patients who come to the emergency room is progressively increasing. The specialization of the physician units might not be adequate for the evaluation of this complexity. The present study aimed to present a standard procedure, called ‘The Geriatric Frailty Network’, operating at the Policlinico Gemelli IRCCS Foundation, which is configured specifically for the level II assessment of frail elderly patients. This was a retrospective study in 1191 patients aged over 65, who were evaluated by the Geriatric Frailty Unit directly after emergency department admission for one year. All patients underwent multidimensional geriatric evaluation. Data were collected on demographics, co-morbidity, disease severity, and Clinical Frailty Scale. Among all patients, 723 were discharged directly from the emergency room with early identification of continuity of care path. Globally, 468 patients were hospitalized with an early assessment of frailty that facilitated the discharge process. The geriatric frailty network model aims to assist the emergency room and ward doctor in the prevention of the most common geriatric syndromes and reduce the number of incongruous hospitalization
